How do I become an approved carrier for TXI?

Approved Carriers will be assigned a Carrier Code; no hauling will be permitted without a carrier code.

To obtain a Carrier Code:

1. Carriers will be required to furnish copies of "Operating Authority"
2. Carriers will be required to furnish an "Insurance Certificate" identifying TXI Operations, LP as certificate holder and showing coverage’s for:
   • (a) Comprehensive General Liability $ 1,000,000
   • Combined single limit for each occurrence, to include bodily injury and property damage, contractual liability and completed operations.
   • (b) Comprehensive Automobile Liability $ 1,000,000
   • Combined single limit per occurrence, to include bodily injury and property damage
   • (c) Workers Compensation and Employers Liability at statutory limits
   • or
   • Accidental Death & Dismemberment/Health Insurance coverage's of:
   • $ 100,000 each accidental death
   • $ 100,000 each loss of limb or sight
   • $ 300,000 medical insurance for each person and 60% pre-injury income for 52 weeks per employee (maximum weekly benefit or $ 400) for loss income.
   • Certificates of Insurance should include a 30 day prior notice of cancellation provision.
3. Carriers will submit a completed Form W-9.
4. Carriers will complete and submitted the following information form:

   Carriers should submit Rate Bids/Billing Terms via fax, email, or US Mail to Etta Dean.
